EVERYTHING YOU NEED TO KNOW ABOUT YOUR DOT EXAM:

DOT stands for Department of Transportation. The Department of Transportation requires a medical examination to make sure you qualify to drive a commercial vehicle in the state of Idaho and throughout the United States.

Upon satisfactory completion of your exam, you will receive a copy of the required MCSA-5875 and MCSA-5876 forms. We will forward a copy of your renewed certification directly to the local Department of Motor Vehicle office for you, as well as enter your information into the National Database.

You should carry your 8-1/2″ x 11″ paper certificate with you at all times while driving your company vehicle, and your employer will want to take a copy for their files as well. Wallet sized copies are available but no longer necessary as your information is available through in the National Database.  DOT certificates are typically issued for 2 years, however, 1 year or less certificates are given for patients with certain health risks.

What to expect during your examination:

We will provide the DOT compliant medical examination form for you to complete. We will also take a copy of your driver’s license for verification upon completion. 

The form will ask you to provide basic information like address, phone, email, whether you are a CDL holder or applying for one with the DMV, as well as other pertinent information.

The form will ask you to provide your past and current medical history and medications, so be sure to have that information with you.

The exam will be performed by our doctors, with the help of their assistant, based on state guidelines and medical standards.  This thorough exam will make sure that you are healthy and help you maintain your medical certification.

During your physical exam, the following tests will be performed and the required forms will be reviewed:

  • VISION:  You will be asked to read a vision chart to assure that you are able to see traffic signs and signals.
  • HEARING:  Please clean your ears before your visit so that Dr. Collin can examine your eardrums.
  • BLOOD PRESSURE:  Your blood pressure must meet state guidelines.
  • URINE:  You will be asked to urinate in a cup so that the doctor can check levels for blood sugar, protein, blood and specific gravity for evidence of underlying health issues. Make sure to drink plenty of water prior to your exam. This is not a drug test. If you are required to get a drug test, there are local offices that can do that for you.
  • SLEEP APNEA required documentation.  If you have been diagnosed with Sleep Apnea and prescribed a CPAP machine, please be sure to bring a print-out of your recent report for the doctor to review and verify.
  • INSULIN required documentation: If you are on insulin, please be prepared to provide an Insulin-Treated Diabetes Mellitus Assessment form signed by your primary physician.
